What we measure
We measure allostatic load. That is the price your body pays for chronic stress.
Most other tests provide disease markers or disease risk markers. We measure something earlier: how hard your body is working to keep up, and how well it still switches off. That shows up in your stress hormones and your autonomic nervous system months or years before it shows up in a standard check-up.
Our standard test kit comprises 4 tests:
- 9 stress hormones in hair: Your long-term stress hormone load over the past three months
- Cortisol saliva profile (2 days): Your daily cortisol rhythm, meaning how sharply you ramp up in the morning and how well you wind down at night.
- ECG monitoring with HRV analysis: Your recovery and acute stress regulation across a workweek
- A scientifically validated self-assessment: Your habits, sleep, gut health, immune status and workplace context.
On top of this standard test kit, we offer add-on tests including:
- Blood test (at-home with Tasso device): Vitamins, minerals, lipids, inflammation markers related to stress regulation
- Glucose monitoring: 10 days of glucose data tracked alongside your stress regulation data
- Microbiome testing: DNA analysis of a stool sample to identify diversity and bacterial composition
- Additional advanced testing like comprehensive fitness and mobility screening (incl. VO2max testing) and other on-demand tests
We compare all of the test results against reference data, compute your Body Reserve Score to represent your allostatic load and give you three to five changes that move your specific bottleneck for allostatic load.
These are the biomarkers included in our standard testing kit:
- Hair stress hormone profile: We measure the past 3 months of stress hormones in 3 cm hair including cortisol, cortisone, DHEA, testosterone, progesterone and five endocannabinoids (AEA, 2-AG, OEA, …).
- Saliva cortisol profile: Six samples across two days measured on waking, 30 minutes after waking, and just before bed. Gives your cortisol awakening response and your diurnal slope, which is how you ramp energy up and down.
- Five-day recovery analytics: A medical-grade ECG monitor worn from Sunday evening to Saturday, day and night. Gives sleep recovery, daytime recovery and stress profile scores.
- Scientific self-assessment: 27+ scientifically validated assessments to provide your health context, habits and evaluate this alongside your biomarkers.
Our add-on tests include these biomarkers:
- At-home blood panel: Lipid panel, inflammation markers (hs-CRP, Albumin), micro-nutrients (B12, B9, D, Ferritin), thyroid hormone (TSH) and upon individual demand further markers
- Continuous glucose monitoring: A ten-day curve with seven individually calculated scores like pre-bed glucose and fasting glucose
- Fitness screen: 4 major mobility patterns, upper and lower body endurance and strength, Y-balance score, VO2max, balance and 6 other scores
- Microbiome analysis: Microbiome balance, microbiome composition and other functional markers
Our standard at-home panel is built around allostatic load, not general screening. It covers hs-CRP, albumin, ferritin, B12, B9, vitamin D, lipids, thyroid hormones and a few others. These are the blood markers that carry the most signal on what stress is doing to you. Additionally you can upload your existing blood test results from the past 12 months.
If you work with one of our doctors longer term, we go deeper. We collaborate with local laboratories for extended panels.
We require head hair to do the long-term stress hormone analysis. If you have less than 1 cm head hair, and as an alternative to hair stress hormone testing, we provide a 3-day saliva sampling protocol that includes extended hormones from saliva like DHEA-S.
Expert support
At least 60 minutes of one-to-one time with a medical doctor, in every program. They walk you through your results, guide you in which two or three things matter, and build your protocol with you.
Two things run through all programs:
- Continuous direct chat access with your preventative medical doctor or expert
- Regular follow-up calls depending on the program you selected
Beyond these standard support options, you can choose the depth of your physician partnership by selecting the program of your choice. You can also contact us for customized support options as well as for being supported by an entire team of experts (e.g., preventative medical doctor, psychiatrist, sleep expert, etc.).
You can select your expert of choice from 9+ preventative medical doctors and experts. These are some of our most selected experts:
- Dr. med. Dominique Sauter-Peschke. Internal medicine doctor who worked at the Klinikum rechts der Isar and has an additional qualification in nutritional medicine. Her clinical background is intensive care, nephrology and cardiology.
- Dr. Lara Maier. Preventive medical doctor and stress medicine specialist, Dr. med. from the Technical University of Munich with additional education in preventative and holistic medicine.
- Dr. Paul Andrä. Preventive psychiatrist and medical doctor, Technische Universität Dresden.
- Dr. Andrea Allgäuer. PhD in immuno-oncology and certified systemic coach. She works on how stress and resilience move the immune system.
- Tarja Mulari. Mindset and performance coach. Olympic gold medalist and former world-record speed skier, with a decade coaching C-suite leaders and boards at UBS, Reckitt, Russell Reynolds and Unilever.
Behind them sit our scientific advisors. Prof. Dr. Clemens Kirschbaum holds the Chair of Biopsychology at TU Dresden and runs the laboratory that analyses your hair and saliva. He pioneered the measurement of cortisol in hair and has published more than 500 peer-reviewed papers. Prof. Robert-Paul Juster of the Université de Montréal advises on allostatic load scoring. Prof. Dr. Rainer Knöller, former Head of Science at the German Athletics Association, advises on how the diagnostics are designed and read.
No. We work upstream, so that your intense lifestyle and workload do not become a medical issue in the first place. If you already see a therapist or a specialist, we work alongside them.
Our health coaches are mostly medical doctors themselves, including psychiatrists. But Sapiens is a wellness offering, not a medical service. We do not diagnose disease, prescribe, or treat. If something in your results points to a medical question, we say so and support you in finding the right expert.
What happens after the results
This is where most other programs stop. We support your habit change long-term in 3 steps:
1) Based on your results and personal goals, your Sapiens expert and you define 3-5 habits each month focused on one intervention area (e.g., sleep optimization, gut health, metabolic health).
2) Then you track those habits daily via a 5 sec WhatsApp prompt we send you every evening. You also log the outcome you care about, whether that is focus, calm, or how you feel after lunch.
3) Your Sapiens expert/preventative medical doctor watches and steps in when you need support. If you fall out of your habits, they reach out. Every month you get a call to clear whatever is genuinely getting in the way, plus follow-up testing to check that the markers moved.
>90% of our clients are still running their habits three months later. At six months it is >80%.
Your kit ships within 48 hours of checkout. You pick any workweek in the following three weeks. Your report arrives within two weeks after your samples reach the laboratory. Then you book your expert debrief to discuss the results.
Effort
Our diagnostics is designed for busy people. It can be done at home in under an hour for the full baseline diagnostics kit, spread across one normal workweek.
- Sunday evening: start the ECG monitor (<5 min)
- Monday and Tuesday: six saliva samples, 30 seconds each.
- Tuesday: hair sample, in under three minutes. Someone else cuts it.
- Any day: 15-minute self-assessment.
- Wednesday: samples collected at your door, or dropped at a post station.
After that it is 60 minutes for the expert debrief and five seconds a day for habit tracking.
The diagnostics is designed for people who travel a lot. While the diagnostics test cannot be taken during a week where you travel for more than 3 hours of time difference, the diagnostics can be done during any other travel week. The ECG monitor can be easily taken off before entering airport security. You can also postpone the test week until a week where you stay at home if that is your preference.
The Allostatic Load Index, which is our core measure, is well published. One recent example: a 2026 study in Communications Medicine (n=1,083) found an allostatic load index matched or beat established biological ageing measures at predicting health outcomes (Vetter et al., 2026: Comprehensive cross-sectional and longitudinal comparison of sixteen markers of biological aging from the Berlin Aging Study II).
Additionally, each individual test is scientifically validated through numerous studies and we are working with a PhD student on the scientific validation of our unique combination of tests and the algorithms behind the Sapiens variation of the allostatic load index that is being used for our reports.
While wearable data are focused on individual markers for sleep and nervous system load, Sapiens measures the price your body pays for stress (especially focused on the drivers and impact of chronic stress).
A wrist or finger sensor gives you a continuous trend of heart rate data as well as HRV data from an optical signal - it can provide a proxy for physiological stress load on one narrow marker.
Sapiens uses a significantly more elaborate panel of whole-body markers covering 7 body systems like gut health, immune health, neuropsychological health, hormonal health (HPA axis) etc.
Three specific differences:
1) We measure hormones. No wearable can give you your three-month cortisol load, your DHEA to cortisol ratio, or your cortisol awakening response.
2) We use a medical-grade ECG instead of the optical proxy signal from a wearable device: That is the reference method your wearable's algorithm is trying to approximate from the wrist. The result is significantly more granular stress and recovery analysis down to the level of how each individual work meeting is driving stress or enabling recovery.
3) You have a preventative medical doctor by your side to read your data and make sense of the complexity of stress and health with you. Together with the comprehensive set of biomarkers, the question that Sapiens is working on is “What’s driving most of my stress?” and not just “How was my sleep last night?” like most wearables.
You can also integrate your wearable data in your Sapiens diagnostics journey to include the trend over time.
Those services run broad blood panels to catch disease early and check micronutrients. That is downstream: what has already gone wrong, or is starting to.
We measure upstream. Stress load and your capacity to regulate it. The hormonal and autonomic systems that decide how well you cope, before it shows up in a lipid panel.
They are complementary, and plenty of our clients do both and then upload their blood test results for the Sapiens diagnostics.
An executive check-up screens for disease and produces a PDF. Useful once a year. It does not measure stress load, and it does not change anything on its own. The depth of the analysis is often limited and is complementary to the testing at Sapiens. You can upload your executive health check-up results with Sapiens.
With Sapiens you get a measurement of allostatic load and nervous system regulation, a protocol built with a preventative medical doctor, and someone who follows up for six or twelve months. The check-up tells you where you stand with regard to specific diseases. Our offering is helping you to identify how well your body is regulating and what drives the physiology of chronic stress. It is also built to change where you stand.
Cost and logistics
You can order individual tests or, our recommendation, select a comprehensive expert-driven program (you find the link on the ‘For Individuals’ Sapiens website):
- Baseline, one-off: Full at-home diagnostics (hair, saliva, five-day ECG), a 15-minute onboarding call, a 60-minute physician consultation, and your personal protocol. Add-on tests available at extra cost.
- Momentum, six months: Extended diagnostics including one add-on test of your choice, three follow-up tests, a 30-minute onboarding call, a 60-minute consultation, six monthly 15-minute coaching calls, and continuous chat with your doctor.
- Resilience, twelve months: Two premium diagnostics with all three add-on tests included, three follow-up tests, a 30-minute onboarding call, two 60-minute consultations, twelve monthly coaching calls, continuous chat with your expert team, and a 90-minute year-end briefing.
Yes. Most of our individual clients pay themselves or expense it afterwards via their employer. There is a "buy as a company" option at checkout if you want it invoiced to your firm instead.
Your data
Your Sapiens expert coach and you.
We pseudonymise all health-related data by default. Your samples and survey answers travel as an encrypted ID, not your name or your email. Third parties, including the laboratories, never receive anything that identifies you. Inside Sapiens, only your designated expert can connect the ID back to you.
No. We never share individual data with an employer. Not your results, not your habits, not anything else about you. If you are part of a company cohort and your team opts in, we can share anonymised group patterns, and only for groups above 8 to 10 people so that nobody can be picked out.
On Microsoft Azure servers in the European Union, under GDPR. Encrypted in transit and at rest. Reports are password-protected and the password reaches you separately by WhatsApp/SMS.
You choose which data to give us. You can request deletion at any time. By default we delete after 24 months.
