Testing Scope
We measure allostatic load. That is the price your body pays for chronic stress.
Most other tests provide disease markers or disease risk markers. We measure something earlier: how hard your body is working to keep up, and how well it still switches off. That shows up in your stress hormones and your autonomic nervous system months or years before it shows up in a standard check-up.
Our standard test kit comprises 4 tests:
- 9 stress hormones in hair: Your long-term stress hormone load over the past three months
- Cortisol saliva profile (2 days): Your daily cortisol rhythm, meaning how sharply you ramp up in the morning and how well you wind down at night.
- ECG monitoring with HRV analysis: Your recovery and acute stress regulation across a workweek
- A scientifically validated self-assessment: Your habits, sleep, gut health, immune status and workplace context.
On top of this standard test kit, we offer add-on tests including:
- Blood test (at-home with Tasso device): Vitamins, minerals, lipids, inflammation markers related to stress regulation
- Glucose monitoring: 10 days of glucose data tracked alongside your stress regulation data
- Microbiome testing: DNA analysis of a stool sample to identify diversity and bacterial composition
- Additional advanced testing like comprehensive fitness and mobility screening (incl. VO2max testing) and other on-demand tests
We compare all of the test results against reference data, compute your Body Reserve Score to represent your allostatic load and give you three to five changes that move your specific bottleneck for allostatic load.
At least 60 minutes of one-to-one time with a medical doctor, in every program. They walk you through your results, guide you in which two or three things matter, and build your protocol with you.
Two things run through all programs:
- Continuous direct chat access with your preventative medical doctor or expert
- Regular follow-up calls depending on the program you selected
Beyond these standard support options, you can choose the depth of your physician partnership by selecting the program of your choice. You can also contact us for customized support options as well as for being supported by an entire team of experts (e.g., preventative medical doctor, psychiatrist, sleep expert, etc.).
Your kit ships within 48 hours of checkout. You pick any workweek in the following three weeks. Your report arrives within two weeks after your samples reach the laboratory. Then you book your expert debrief to discuss the results.
Our diagnostics is designed for busy people. It can be done at home in under an hour for the full baseline diagnostics kit, spread across one normal workweek.
- Sunday evening: start the ECG monitor (<5 min)
- Monday and Tuesday: six saliva samples, 30 seconds each.
- Tuesday: hair sample, in under three minutes. Someone else cuts it.
- Any day: 15-minute self-assessment.
- Wednesday: samples collected at your door, or dropped at a post station.
After that it is 60 minutes for the expert debrief and five seconds a day for habit tracking.
Market Comparison
While wearable data are focused on individual markers for sleep and nervous system load, Sapiens measures the price your body pays for stress (especially focused on the drivers and impact of chronic stress).
A wrist or finger sensor gives you a continuous trend of heart rate data as well as HRV data from an optical signal - it can provide a proxy for physiological stress load on one narrow marker.
Sapiens uses a significantly more elaborate panel of whole-body markers covering 7 body systems like gut health, immune health, neuropsychological health, hormonal health (HPA axis) etc.
Three specific differences:
1) We measure hormones. No wearable can give you your three-month cortisol load, your DHEA to cortisol ratio, or your cortisol awakening response.
2) We use a medical-grade ECG instead of the optical proxy signal from a wearable device: That is the reference method your wearable's algorithm is trying to approximate from the wrist. The result is significantly more granular stress and recovery analysis down to the level of how each individual work meeting is driving stress or enabling recovery.
3) You have a preventative medical doctor by your side to read your data and make sense of the complexity of stress and health with you. Together with the comprehensive set of biomarkers, the question that Sapiens is working on is “What’s driving most of my stress?” and not just “How was my sleep last night?” like most wearables.
You can also integrate your wearable data in your Sapiens diagnostics journey to include the trend over time.
Those services run broad blood panels to catch disease early and check micronutrients. That is downstream: what has already gone wrong, or is starting to.
We measure upstream. Stress load and your capacity to regulate it. The hormonal and autonomic systems that decide how well you cope, before it shows up in a lipid panel.
They are complementary, and plenty of our clients do both and then upload their blood test results for the Sapiens diagnostics.
An executive check-up screens for disease and produces a PDF. Useful once a year. It does not measure stress load, and it does not change anything on its own. The depth of the analysis is often limited and is complementary to the testing at Sapiens. You can upload your executive health check-up results with Sapiens.
With Sapiens you get a measurement of allostatic load and nervous system regulation, a protocol built with a preventative medical doctor, and someone who follows up for six or twelve months. The check-up tells you where you stand with regard to specific diseases. Our offering is helping you to identify how well your body is regulating and what drives the physiology of chronic stress.
